Eligibility:    

• The New Leaf Online Peloton Bike Giveaway (the "Giveaway") is open only to legal residents of Alabama, Georgia, Florida, and Texas who are 18 years of age or older at the time of entry. 

• Employees, officers, and directors of Auburn Direct Primary Care, LLC (doing business as "New Leaf Online") ("Sponsor"), its parent company, subsidiaries, affiliates, advertising and promotion agencies, and their immediate family members (spouse, parents, children, siblings, and their respective spouses) and those living in the same household of each are not eligible.    

• Void where prohibited by law. 

2. Giveaway Period:

The Giveaway begins at 12:00 AM Central Time ("CT") on April 7, 2025, and ends at 11:59 PM CT on May 7, 2025 (the "Giveaway Period"). 

3. How to Enter:

There are three ways to enter the Giveaway: 

• Enroll: Enroll in the New Leaf Online Medical Weight Loss program during the Giveaway Period. 

• Refer: Refer a friend who enrolls in the New Leaf Online Medical Weight Loss program during the Giveaway Period. [Include specific instructions on how to make a referral, e.g., "To refer a friend, provide them with your unique referral link, which can be found in your patient portal."] 

• Mail-In Entry: Hand-print your full name, complete address, phone number, and email address on a 3″ x 5″ card and mail it in a #10 envelope with proper postage to: New Leaf Online Peloton Bike Giveaway, 440 N Dean Rd, Auburn, AL 36830. Mail-in entries must be postmarked by May 7, 2025, and received by May 12, 2025. 

4. Entry Limit:

There is no limit to the number of entries you can earn through referrals. However, you can only enroll in the program once and submit one mail-in entry. 

5. Prize:

One (1) winner will receive a Peloton Bike (approximate retail value: $[Current Retail Value of Peloton Bike]). The prize is non-transferable and no cash alternative will be offered. 

6. Winner Selection and Notification:

The winner will be selected in a random drawing from all eligible entries received on or about May 16, 2025. The winner will be notified by phone and email on or before May 20, 2025. If the winner cannot be contacted or does not respond within three (3) business days of notification, an alternate winner may be selected. 

7. General Conditions:

• By entering the Giveaway, you agree to be bound by these Official Rules and the decisions of the Sponsor, which are final and binding in all matters relating to the Giveaway.    

• The Sponsor is not responsible for lost, late, incomplete, invalid, unintelligible, or misdirected entries, which will be disqualified.    

• The Sponsor reserves the right to cancel, suspend, or modify the Giveaway if fraud, technical failures, or any other factor beyond the Sponsor's reasonable control impairs the integrity of the Giveaway, as determined by the Sponsor in its sole discretion. 

• All federal, state, and local laws and regulations apply. 

8. Release and Limitations of Liability:

• By entering the Giveaway, you release and hold harmless the Sponsor, its parent company, subsidiaries, affiliates, advertising and promotion agencies, and their respective officers, directors, employees, and agents from any and all liability for any injuries, losses, or damages of any kind arising from or in connection with the Giveaway or acceptance, possession, or use of the prize.    

• The Sponsor's liability for any claims arising out of or relating to the Giveaway shall be limited to the cost of entering the Giveaway, if any. 

9. Sponsor:

The Giveaway is sponsored by Auburn Direct Primary Care, LLC, doing business as New Leaf Online, 440 N Dean Rd, Auburn, AL 36830. 

10. Winner's List:

For the name of the winner, send a self-addressed, stamped envelope to: New Leaf Online Peloton Bike Giveaway Winner, 440 N Dean Rd, Auburn, AL 36830. Requests must be received by February 16, 2025. 

11. Privacy:

Any personal information collected from entrants will be used by Auburn Direct Primary Care, LLC for the purpose of administering the Giveaway and for other purposes as outlined in the Sponsor's privacy policy, available at [link to your privacy policy]. 

12. Disclaimer:

These rules are intended to comply with the laws of Alabama, Florida, Georgia, and Texas. In the event of any conflict between these rules and applicable law, the law shall prevail.
